Ministry Request Form (Blue Sheet)
If you have an event, meeting or other activity that you want to add to the church calendar, please complete a Ministry Request Form. We call them “blue sheets.” Click here for a fillable PDF that you can complete and email to Stephen Miller. Our staff will review your request in our staff meeting to make sure there are no conflicts on our calendar.
